JOINT AFFIDAVIT OF TWO DISINTERESTED PERSON

WE, JOSELITO A. PESCO and LUCITA L. BAO residents of Barangay

Manapa, Buenavista, Agusan del Norte, both legal age, Filipinos, after having
been duly sworn to in accordance with law, do hereby depose and state:

1. That we personally know MRS. TERESITA GERMO GULAY

PABILAN as the lawful wife of JUANITO VELASCO PABILAN who died
recently;

2. That the deceased Mr. Pabilan based on Social Security System

Self-Employed Data Record (SSS) RS-1 No. 0821190423 Encoded on
December 16, 1996 identified as his beneficiaries his spouse Teresita and
her children with PEDRO APDUHAN AMOY, out of wedlock, namely,
Rolando, Renato, Renante and Theresa, all surnamed Amoy;

2.1. Teresita and Pedro were never married as

evidenced by a Certificate of No Marriage issued by the Philippine
Statistics Authority (details).....

3. That erroneously entered in the deceased’ above-mentioned SSS

RS-1, it was THERESA G. Amoy instead of TERESA G. Amoy, the latter
being her correct name based on her Certification Live of Birth No. 00201-
A83DM01-1;

4. That, also, erroneously entered in the deceased’ above-

mentioned SSS RS-1, was VALERIANA VELASCO as her biological
mother, instead of BALERIANA VELASCO;

5. That We personally know the circumstances of the persons we

mentioned above because we are family friends for several years up to the
present;

6. That we are executing this affidavit in order to correct the minor

discrepancies in the SSS RS-1 of the deceased Juanito V. Pabilan related
to the names of TERESA G. AMOY and MRS. BALERIANA V. PABILAN,
to attest to the truth of the facts aforementioned, and, for any legal purpose
this may serve.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 31st

day of July 2023 at Buenavista, Agusan del Norte, Philippines.
