    I am Jonathan D. Apostol with an ID number 2190377, one of your good student requesting to authorize
    Ms. Maria Ruffa E. De Vera to request and pick-up the following:

     2nd Semester 2019-2020 Grade


     1st Semester 2020-2021 Enrollment Form
     1st Semester 2020-2021 Grade
     2nd Semester 2020-2021 Enrollment Form

    For I need those stated documents for my scholarship in CHED under the Tulong Dunong Program.

    I cannot personally go to request and pick-up my said records because I am here in my province and
    cannot go to Baguio City right now because of our situation.
    I am also authorizing Ms. De Vera to sign any documents pertaining to my records that would help to
    release my requested records immediately.
