1.0 PURPOSE
The purpose of this procedure is to ensure all risks associated with working at height within the
operations of Multi AMB Limited is carried out in a safe manner, to prevent employees from being
injured from falls.

2.0 SCOPE OF APPLICATION


This procedure applies to all tasks that require working at a height which are performed in Multi AMB
Limited and any other location where a Multi AMB employee would be undertaking such a task, or
any other contractor working on behalf of Multi AMB Limited.
It covers any work at height above 6 feet or (1.8m) of relative/reference surface/floor.

3.1 RESPONSIBILITIES
Managing Director
The Managing Director will approve of a Permit to Work for commencement of work and ensure
that any work at height is carried out with safe equipment, in a safe place following a safe process
with trained and competent people.

Heads of Departments/ Supervisors


The heads of departments and supervisors will ensure all the mandatory checks/test have been done
and a Permit to Work issued. Control personnel throughout the working at height activity and ensure
the presence of a full time observer for the duration of work.

HSE Officer

 Ensure the use of appropriate Personal Protective Equipment's /Clothing's (PPE's/PPC's);


 Identifying and classifying height permitted locations/tasks;
 Identifying working at height hazards within the workplace;
 Consulting Supervisor and employees in relation to identifying and managing working at height
hazards;
 Ensuring risk assessments are conducted for identified working at height tasks;
 Reviewing controls for working at height risks for effectiveness;
 Issuing a Permit to Work to a contractor or employee working at height at 6 feet or (1.8m)
above or below ground level;
 Complying with legislation relating to working at height.

4.0 PROCEDURE

A. Identification of Fall Hazards


The Supervisor, HSE Officer or his representative must determine if the personnel will be
exposed to the hazard of falling from a work area that is;
 1.8 metres or more above the nearest safe surface or below;
 Above a surface or thing that could cause injury to the worker if he/she were to fall on the surface
or thing; or
 Above an open tank or pit containing hazardous materials.

B. Hazard Assessment
A hazard assessment must be carried out to identify the hazards associated with working at height and
the control measures needed to deal with those hazards. The result of that assessment is to be written
down on the risk assessment form and communicated to all those at risk from the hazard.

If a worker is exposed to any fall hazards, then adequate work platforms or staging must be
provided where it is practical to do so, otherwise at least one of the following
control measures must be implemented:

 Guardrail is constructed or installed.


 Height Personnel is provided with a fall restraint or arrest system.
 Height Personnel safety net is installed.
 Temporary flooring is constructed or installed.
 Height Personnel is provided with another means of fall protection.
 No work should be carried out at height where it is reasonable practical to carry out that work at
ground level.
 Adequate precautions must be taken to prevent any person falling a distance liable to cause personal
injury.

C. Guardrail
 Guardrails must be installed where an open-sided surface is over 1.8 metres above the floor or
ground, a stairway ends in direct proximity to dangerous traffic or other hazards and on walkways
over open tanks containing harmful substances or over open tanks.
 Ensure the guardrails are secured so they cannot be moved in any direction.
 If a portion of a guardrail has to be temporarily removed then the exposed height personnel must be
protected by another fall protection system.

D. Fall Arrest System


 Height Personnel must determine and assemble the necessary components.
 Height Personnel must calculate the total fall distance and ensure that an adequate clearance
distance is available.
 Height Personnel must ensure that the harness is adjusted to fit properly and is rated for his/her
weight.
 A full body harness and appropriate lanyard must be secured to an anchorage point, lifeline or static
line.
 Only one Height Personnel is permitted to be attached to a lifeline at any one time.
 A lifeline must be provided with softeners at all sharp edges or corners.

E. Personnel Safety Net


 A personnel safety net must be installed no more than 4 metres below the work area, extend 2
metres on all sides beyond the work area and be maintained so a fallen Height Personnel does not
contact another surface (Subject to specific work area).

F. Other Means of Fall Protection


 Other means of fall protection can include a harness, net, rope, body belt, structure or other
equipment, device or means.
 A fall restraint system must be arranged so a worker cannot fall lower than the surface on which the
worker was supported on.

G. Work Permit

All work at height or at ground level where an employee or contractor could fall into an excavation,

manhole or from an equipment or machinery must be carried out under the control of a Work Permit.
Multi AMB Limited employees or contractors undertaking such work must seek the issue of a Work
Permit from the HSE Officer. No work at height is permitted until the Work Permit is issued.
