1. We regretfully inform you that your certification shown in Table A below will be cancelled and
removed from both IIC (www.iicregistrar.com) and JAS-ANZ registrar (www.jas-anz.org) in
accordance with ISO/IEC 17021/9.6.5.5 and AP-09/8.2.2; no immediate remedy to resolve the
standing issues that have resulted in the suspension exceeding 2 months after the notification
of suspension.

2. Upon your certification withdrawal, you are not allowed to use any certification marks or
statements that might mislead other clients that you are still certified to the relevant Standard.
Please be aware that any contravention of the withdrawal conditions set forth in F06-5-1
Certification Audit Agreement you have signed could result in legal claim.

3. We advise you to return the original certificate issued by us or raise objections within 7 days
upon the receipt of this notification. If you have any appeal on our decision, please write us in
above same days.

4. It is unfortunate that the agreement did not work out to our mutual benefits, but we deeply
appreciate of your past cooperation to maintain the certification with us and wish you well in all
your future endeavors. If you need our services in the future again, please do not hesitate to
