Loss and Damage Policy

IT & OFFICE EQUIPMENT AGREEMENT

I, Jonathan understand that the below equipment is owned by and I am using it for work-related purposes. I
understand that I am responsible for the care and maintenance of the equipment. If the equipment should be,
damaged, lost, stolen, while outside the office premises, I shall be responsible for the cost for its replacement or
repairs. In case the equipment, gets broken due to wear and tear, I shall have it inspected only by the
company’s technical department (not a 3rd party repairer) as soon as possible.

I understand that upon the termination of my employment, I shall surrender and return all equipment back to the
company. In absence of notice, the company has the right to withhold my final pay until all the equipment noted
below has been returned.

By signing this Agreement, I acknowledge that I have received the office equipment described below in working
order and I confirm to the terms and conditions for the use of said equipment as stated in this agreement.
