ACCOMPLISHMENT REPORT FOR REMEDIAL ACTIVITIES

GRADE FIVE
S.Y 2021-2022

I. INTRODUCTION

Department of Education never ceases of creating moves to achieve a quality

education that will help augment its prepared plans, programs, and policies in
different subject areas to the needs of the present technological era. Each school,
together with its internal and external stakeholders must show full support for the
success of the designed curriculum made by the planners of the Department.

Supporting the plans and goals of the Department, internal and external
stakeholders of Calantas Elementary School are moving together to achieve what is
ought to be set by the division inline with mathematics which is parallel to the
department’s plan.

Remedial sessions are needed to uplift the pupils who hardly deal with
mathematical problems involving the four fundamental operations.

II. ACTIVITIES

THE TEACHER:

a. Prepared Remedial lesson plan for Mathematics

b. Crafted teacher-made-worksheet for the remedial sessions
c. Conducted remedial class before and after class hours
d. Assessed learners through written test crafted by the teacher
e. Constructed intervention material and related activity/ worksheets in
mathematics.
