ANNEX B- STUDENT TRAINEE ON THE JOB TRAINING AGREEMENT

Agreement made for Name of Student with the residence and postal address at
Student Address here in referred to as OJT Trainee who will undergo training at
Name of Company and Address with the duration of Training Hours.

That the OJT TRAINEE shall undertake the following:

1. Shall be personally responsible for any and all liabilities arising from negligence in the
performance of his/her duties and functions while under training;

2. Comply with the requirements and guidelines of the OJT program;

3. Submit all the requirements and/or portfolios for the OJT program not limited to Progress
Reports, Final Output, etc.

4. Observe and follow all the unit’s rules, policies, code of conduct, code of discipline, code of
ethics, safety procedures, and the likes, for the effective, safe and successful completion of
the program;

5. Regularly report to the company for the practicum work and notify the immediate superior
or any key personnel in case of inability to do so for any reason whatsoever;

6. To exercise care, due diligence and safety in using all company resources and to return any
company property issued in good condition upon the completion of the OJT;

7. To maintain privileged information or any matters obtained during the course of the OJT in
line with the company and national security and interest;

8. Reasonably perform OJT/trainee related tasks, duties and responsibilities assigned by the
immediate superior/ training supervisor;

9. Whenever applicable, for trainings beyond regular hours / schedule, to notify or present the
approval of the parent and/or program OJT coordinator to the company.
