for Survey Authority and authorizing certain private Geodetic Engineer to
conduct the survey

 Duly Accomplished Letter Request Form from Geodetic Engineer on behalf
on his/her client

 Survey Authority Form duly signed by the applicant and private GE
(1 original 1 duplicate)
 Updated Tax Declaration (1 certified copy)
 Barangay Certification (1 original 1 duplicate)
 LRA Certification (if the municipality has undergone cadastral proceedings
or if there is an old survey) (1 original 1 Duplicate)
 RTC Certification (1 original)
 Any proof of claim or acquisition of the subject lot (1 original)
 Extrajudicial Settlement of Estate
 Deed of Donation
 Deed of Sale
 Certificate of Land Status (1 original)
 Alienable & Disposable Certification
 Scheme of Subdivision from GE (1 photocopy)
 Copy of Approved plan with Technical Description (if with previously
approved survey) 1 blueprint copy
 Certified copy of Cadastral Map and Lot Data Computation as reference
 Geo-tagged photos on subject lot with applicant and Inspector/ Investigator
 Inspection Fee 200.00
 Cadastral Cost Depending on Municipality
