INCIDENT DESCRIPTION
On 5th November 2019, a Sandvik service specialist was travelling to a mine site on a customer
service vehicle. While approaching the mine site, the front right suspension lower arm joint broken.
At the time of incident, the vehicle was moving at 20 Kilometres per hour speed. The vehicle with
driver was provided by customer for picking up Sandvik service specialist from guesthouse
(approximately 12 kilometres from mine site). No injuries reported.

IMMEDIATE CAUSES
Using unsafe or defective plant/equipment: Using vehicle which was in unsafe

condition.
Inadequate pre-start inspection conducted: Daily pre-start inspection of customer
vehicle was not done and Sandvik engineer did not check the condition of vehicle as it is

provided for one-time use (one single trip to a shut-down plant for collecting a spare
part).

ROOT CAUSES
Inadequate maintenance undertaken; Customer did not have procedures/process to

ensure safe use of passenger vehicles.
Inadequate monitoring of compliance with EHS policy, standards, procedures or work
2 instructions; Sandvik service engineer did not realise the need of using vehicle check
sheet considering that this vehicle trip is for a single use.

ABSENT OR FAILED CONTROLS / SUPPORTING ACTIVITIES
