CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This document executed by and between Sangguniang Barangay of Barangay Bangac

represented by PUNONG BARANGAY LORENZO M. PATANI JR., of legal age, Filipino, married and
resident of Barangay Bangac Mina, Iloilo herein after referred to as the FIRST PARTY;

AMADEUS Catering Services, Mina, Iloilo represented by RAZEL JADE PEREZ , owner, of legal age
and residents of Brgy. Mina ,Mina, Iloilo City, herein referred to as the SECOND PARTY:

WHEREAS, the FIRST PARTY desire for the service of Catering Service for the barangay during of
BARANGAY DISASTER RISK REDUCTION AND MANAGEMENT of Barangay Bangac Municipality of
Mina;

WHEREAS, the SECOND PARTY has expressed willingness to render the Services as hired by the
FIRST PARTY on a contract of service basis;

NOW THEREFORE, for and in consideration of the foregoing, the FIRST PARTY hereby engages
the services of the SECOND PARTY under the following terms and conditions:

LUNCH: Php. 232.5.00/ pack x 38 pack = Php. 8,835.00

SNACKS: Php. 100.00/ pack x 38 pack = Php. 3,800.00

That the FIRST PARTY Shall pay the fee as charged by the SECOND PARTY in the amount TWELVE
THOUSAND SIX HUNDRED THIRTY FIVE PESOS ONLY (Php. 12,635.00) to render service for the Catering
Service rental of barangay Bangac of the Municipality.

That SECOND PARTY shall amount of Php. 12,635.00 and shall issue an official receipt to the FIRST
PARTY as a proof of such payment.

IN WITNESS WHEREOF, the parties have here under affixed their signature this 25 th date of January, 2021.

BEFORE ME, a Punong Barangay, Barangay Bangac, Municipality of Mina, Province of Iloilo, Philippines ,

Community tax Certificates/ valid identification card indicate below their respective names known to me be the
same persons who executed the foregoing instrument and acknowledged to me that the same is their fee and
voluntary act and deed.
