Sir:
I have the honor to request for the separation of Trece Martires City
National High School – Conchu Annex from Trece Martires City National High
School – Main and be it named as Agapito Conchu National High School in
line with the following reasons:
1. This annex has a complete four levels from Grade Seven to Grade Ten;
2. Teachers are trained and well equipped in providing quality
education;
3. The school has adequate school facilities, books and other
instructional materials for better teaching and learning of students;
4. Every year, there is a vast increase of enrollment, from its first
operation of male and female for SY 2009 – 2010 to male and female
this SY 2022-2023.
5. It has its own funding requirements;
6. Lastly, it is a demand of the community for identity.

III. Description of School Site and School Building
In the year 2009, the Adelina Realty Corporation has donated a portion
of land to the City Government of Trece Martires City with a total area of 4,293
square meters, located at Block 43 Lot 5 – A Ciudad Adelina Subdivision with
TCT No. 423893 wherein the high school campus of Trece Martires City
National High School – Conchu Annex is located and has been constructed.
1 unit two ( 2 ) storey building with four classrooms, (DepEd Building), 1 unit
two ( 2 ) storey building with 4 classrooms, ( LGU Building ), 2 classrooms were
use for ICT laboratory. 1 unit 2 classrooms ( FFCCCII Building )
, and a building with 6 rooms were constructed in the area. 3 rooms were used
for grade seven classrooms, other rooms function as library, faculty and
guidance room. The School has its newly constructed fence through the
initiative of Gov. Remulla and a school covered court through Congressman
Luis Ferrer.
The school has complete four levels from grade 7 to grade 10. All the
teachers are trained and well equip in providing quality education, then
schools has adequate school facilities, books and other instructional materials
for teaching and learning processes.
The school surroundings is very conducive to learning, each classroom
has its own LCD/Led television for instruction and lastly, school buildings are
all in good condition.
For the last five ( 5 ) years of school existence, it has a total enrollment
of:

Every year there is a vast increase of enrolment, and lastly the school
has its own funding requirements and the separation from its mother school is
the demand of the community.
JUSTIFICATION
Justification is hereby issued in favour of Trece Martires City National High School – Conchu
Annex on the need to separate from the mother school Trece Martires City National High School
– Main. The following will atleast the reason for separation:
1. This annex has a complete four levels from Grade Seven to Grade 10.
2. Teachers are trained and well equipped in providing quality education.
3. The school has adequate school facilities, books and other instructional materials for
better teaching and better learning of students.
4. Every year there is a vast increase of enrollment.
5. It has its own funding requirements.
6. Lastly it is the demand of the community.

Conchu Annex on its establishment, that Trece Martires City National High School – Conchu
Annex was established as a school annex of Trece Martires City National High School on
November 5, 2009 with School identification Number 307818, duly approved by DepEd
Regional Director Dr. Lorna Dig Dinio.
The Approved letter of establishment was nowhere to found.
That Trece Martires City National High School – Conchu Annex was operational with
307818 as School ID from SY 2009-2010 to present school year.
