Further, I hereby affirm by my signature not to disclose any personal data extracted and
processed to a third party, either within or outside the Office, without permission from the Head
of the Office.

I am fully aware that any unauthorized release or negligence in the handling of personal
information is considered a breach of my duty to maintain confidentiality, which could serve as
grounds for immediate dismissal and/or possible personal liability, pursuant to Republic Act No.
10173 or the Data Privacy Act of 2012, or in any legal action arising from such breach.
