AFFIDAVIT

That I ANACLETO G. JABONETE Filipino, of legal age, married,

and a resident of Brgy Iloilo City, after having been duly sworn in
accordance with law, hereby depose and state:

1. That I am familiar to the Certificate of Live Birth of ZERAH

JENICA L. JABONETE which was made to appear to have
been born on March 25, 2018 in Brgy. Bungca, Barotac Nuevo,
Iloilo and whose birth was duly attended by a “Hilot”;

2. That I am the same person referred as the “Father” in the said

Certificate of Live Birth;

3. That under oath, I hereby declare that the said Certificate of Live
birth of ZERAH JENICA L. JABONETE fictitious certificate
inasmuch as all the contents the contents thereof are not correct;

4. That I was only made to sign the said Certificate of Live Birth by
my wife JENISSA L. JABONETE as she claimed that she will
be adopting the said child;

5. That it is not true that JENISSA L. JABONETE gave birth to a

child on March 25, 2023 at Brgy. Bungca, Barotac Nuevo, Iloilo,
and the truth is that, the child referred in the said Certificate of
Live Birth is the child of ANE M. PESTAÑO who was born on
March 25, 2018 at Western Visayas Medical Center and whom
they called as “NIKKA PESTAÑO”;

6. That I am executing this affidavit to attest to the truth of the

foregoing statements and for whatever legal purpose it may
serve.
