Notes and Suggestions for Instructors
General
You might find teaching easier if you write the answers in your copy of the textbook. Then look them up in the
references (CPT, HCPCS, ICD-10-CM, or ICD-9-CM) and prepare for student questions about the correct
answers. You may wish to note the key word in the index, the terms that could cause confusion about the
answer, or other questions the class may ask about the item.
One of the common errors students make is selecting a code out of the range specified. This is further
reinforcement of the need to read carefully ALL instructions before selecting an answer. The CPT index does
not include all the terms used in the CPT code descriptions. If your students have good medical terminology
skills, they may find it easier to go to the specified code range, determine where the service would appear, and
select the answer without using the index.
The ICD-10-CM reference this year was the 2015 draft. However, since codes are generally “frozen,” it may be
possible to use whatever edition of ICD-10-CM you have available.
There may be changes in the final ICD-10-CM when it becomes available; however, the structure should remain
the same. When I used these materials in the classroom, I went around the room and had each student pronounce
the term and give the code. We paused briefly for any codes other students may have selected. If my answer was
a different code, I gave the students credit for their code if they could defend their selection and establish it as
correct. Very few of their codes met this standard, but this exercise offered insight into incorrect conclusions in
the coding thought process. We would discuss each item to be certain all students had the correct answer.
Because I started at a different point in the room each class period, the students could not be sure they would
always have number 11 or any other regular pattern. If the student did not have the answer to the item, he or she
said “pass” and the next student offered the code. However, the next item went back to the previous student. If
that student said “pass” to items 16, 17, 18, and 19, it was evident he or she had not completed the worksheet.
This happened no more than once each semester.
I encouraged the students to do their work on notebook paper, entering only the correct codes in their textbooks.
This kept the book neat, so they could use the pages from the appropriate sections as part of their portfolio on
future job interviews. When seeking a position in a urology office, for example, they could take the CPT and
ICD-10/9-CM coded pages to show familiarity with the procedures and diagnoses of that specialty. By including
both ICD-10/9-CM codes in this workbook, students will be able to impress potential employers with their
exposure to and ability to work in both coding references.

Coding and Reimbursement


This workbook codes the CPT, HCPCS, and ICD-10/9-CM references “by the book.” It is important that
students learn correct coding procedures before trying to understand how the various payers bend or break the
rules. Just as we all read the driver training materials and are meticulous about not violating any rule while
taking our driver’s test, we soon learn that those rules may fail us on the “real” road. Generally, when any payer
(Medicare, managed care plan, or Worker’s Compensation) deviates from the rules in the coding references, we
can find this change in the payer’s manuals, newsletters, or other publications.

As an example, the Medicaid manual may state that removing an IUD is included in the appropriate level of
office visit (992xx). That means that the code for removing an IUD (58301) is not a Medicaid benefit. If the
office reports code 58301, Medicaid will reject the service, stating it is not a Medicaid benefit, or perhaps the
rejection will indicate that the service is included in another service. In no way does this payer restriction change
the fact that the correct codes are 992xx and 58301 for these services. It is just a reporting rule or requirement of
the Medicaid program.
Many factors influence the payment for a service. Certainly the physician must code it correctly and follow the
payer’s reporting instructions. However, the terms of the patient’s policy are the final arbiter of payment. We
understand that cosmetic surgery will not be paid and do not expect reimbursement from the patient’s insurance
for a facelift, a “nose job,” or a hair transplant. Some groups may exclude services on religious grounds
(pregnancy terminations or sterilizations), limit payment for some diagnoses (pay up to $600.00 a year for
obesity treatment), or require that all laboratory services be done at a specified lab. Precise coding will not
secure payment from these plans if it is the elective abortion, the balance of the $2000.00 obesity treatment, or
the lab work done in the doctor’s office. Other policy restrictions may also apply. Failure to obtain prior
authorization, a second opinion, or other pre-procedure requirement may render the service ineligible for
payment.

CPT
The Evaluation and Management code section is often confusing, so you may wish to do this unit last. It does
not follow the standard coding rules and students may do better if they begin with the surgery section.
The Supervision and Interpretation (S&I) codes in the Radiology sections probably need to be reported to local
payers with the modifier –26. A few payers may determine it to be the professional component just because the
place of service is hospital outpatient or inpatient and the doctor did not use his own equipment. You may elect
to accept the codes with or without the –26 modifier, but notify the students in advance of your decision. I show
codes with the –26 modifier, a “professional component” service, as a reminder that –26 may be necessary. The
–RT and –LT modifiers are attached to many services, as many payers now expect them. Since modifiers that
affect payment are reported first, the modifiers should be –26-LT, as the –26 modifier reduces the payment.
It is difficult to find many of the worksheet items in the CPT index. Coders should look up in the index the
codes that come closest to the description provided in the item. If that does not provide the correct answer, it
may provide a clue to other terms to look for in the index. Sometimes it may be faster to go to the CPT code
section and avoid the index. This is especially true in the musculoskeletal system where there are many ways to
say the same thing. If you have only one or two students who find the correct code, ask them to share their
approach for finding it with the class.

ICD-10-CM and ICD-9-CM


Decimal points appear in the worksheet answers, but some payer rules may prohibit the use of punctuation and
most computer systems strip them off before the claim can be filed. For other payers, punctuation can cause the
claim form scanners to misread the codes. However, it is recommended that physician’s office coders get in the
habit of including the decimal point in any diagnosis code as the seven-character format for ICD-10-CM is
easier to read with the decimal point. Also for easier reading, I show the ICD-10-CM place holder “X” in lower
case.
