Attached herewith are the series of Deed of Sales the latest of which is between
Irenio Cambarijan and me, Karen Dutosme Duay as the VENDEE together with
Certified electronic copy of Original Certificate of Title no.P-19119 (P-3171) and my
narrative Affidavit for a particular parcel of land situated in the Barrio of Lapla (formerly
Dalay) Malalag, (formerly part of Sulop ) and known as Lot no. 228, Pls-632 containing
an area of 61, 581 square meters.

In your Office records, the present declarant is Manuel Campaner with entries the
same as the entries in OCT no. P-19119 ) P-3171 registered in the name of AMBIT
TAMBIS as to lot number, survey number and boundaries.

Please be informed that it is my intention to transfer the title registered in the

name of AMBIT TAMBIS to my name by virtue of the series of Deed of Absolute Sale
mentioned above. In order to effect the transfer of OCT no. P-19119)P-3171 to my name,
I was informed by the Office of the Register of Deeds, Bureau of Internal Revenue and
the Department of Agrarian Office that the declarant’s in the Tax declaration with its
entries and data should be the same with that of the name in OCT no. (P-19119) P-3171.

Hence, it is respectfully requested that your Office can produce a tax declaration
in the name of Ambit Tambis and cancel the tax declaration of Manuel Campaner based
on the dcuments attached and enumerated hereunder to wit;

2. Certified Copy of Deed of Absolute Sale from Ambit Tambis to Manuel Campaner.
3. Certified Copy of Deed of Absolute Sale from Manuel Campaner to Irenio
Cambarijan.
4. Certified Copy of Deed of Absolute Sale from Irenio Cambarijan to me, Karen
Dutorme Duay.
5. Certified electronic copy of OCT no. (P-19119) P-3171.
6. Tax declaration of Manuel Campaner (photocopy).
7. Updated Tax receipt of Manuel Campaner (photocopy).
