CHAPTER 1
INTRODUCTION

Nursing has always been used synonymously and interchangeably with caring and

promoting comfort. Nurses are those who are responsible for maintaining and

ensuring that the highest quality of care is rendered. However, the responsibilities

and workload of nurses are not only limited to bedside care, but it is also far more

complicated. They are in the front line, tending to the sick throughout the shift,

counseling and educating patients, functioning as health advocates, manager, and

supervisor. In their daily lives, nurses are faced and bombarded with tasks of

improving and developing and strengthening professional competence and

commitment. The challenge is that they are expected and required to maintain the

highest of care despite these said tasks and responsibilities the nurses are carrying

in their shoulders. The roles of nurses are becoming more visible, appreciated, and

addressed during times of scarcity. Even though nursing is one of the largest groups

constituting health care professionals, nursing vacancy and shortage is still

prevalent. A nursing shortage occurs when the demand for competent nursing

professionals exceeds the supply of qualified nurses. Ironically, as the top exporter of

nurses in the world, the health care setting in this country is still facing the challenge

of this crisis.

3
DEFINING THE CENTRAL PROBLEM

The issue of a nurse shortage is not new to the healthcare industry, and there are

numerous, serious causes for concern. Furthermore, it burns in critical and intensive

care units, where the strain is tremendous and nurses are at a higher risk of burnout.

Nurse shortages have a detrimental influence on more than just nurses. It also has

an impact on patients and can result in unfavorable patient outcomes due to a

nurse's lack of attention to a patient.

Allied Care Experts (ACE) Medical Center - Tacloban, Inc. is a respected Level 2

Private Hospital that serves a diversified population in an urban area that extends

throughout Region VIII, which includes six provinces. Due to variables such as rising

patient admissions, high turnover rates, a lack of potential educators, and a

restricted pool of available nurses in the region, the hospital has struggled with a

shortage of registered nurses (RNs) over the past few years. This deficit has resulted

in compromised patient care and exhaustion among current nursing staff. The

hospital's Nursing administration is aggressively looking for appropriate solutions to

this situation.

nurses, trainers, and educators– whether inhouse or external sources that will deliver

inspiration and powerful workmanship amongst the nurse community in ACE Medical

Center. To cultivate a compassionate, trustworthy and patient-centered service that

will bring pride and achievement to its respective units or wards. To build a band of

hardworking individuals which aims at the CORE VALUES of ACE– Altruism,

Compassion and Excellence.

WEIGHING THE OPTIONS AND ALTERNATIVES

Alternative Solution: Establishing a Nurse Readiness Program (NRP)

Overview

As an alternative solution to the nurse shortage, the hospital might establish a

thorough nurse readiness program. Nurse residency programs are planned,

supporting efforts that assist freshly minted nurses in seamlessly transitioning into

professional practice. This approach, however, would be modified to include

experienced nurses as well, with the goal of both attracting new nurses and retaining

those who are already in the field.

8
Detailed Implementation

Program Development:

● Work with nursing schools and colleges to develop a structured curriculum

that combines classroom instruction with hands-on clinical experiences.

● Offer several tracks to tailor the training to the individual needs of fresh

graduates and seasoned nurses.

● Create a curriculum that focuses on critical thinking, decision-making, and

specialist areas pertinent to the hospital's services.

Preceptorship and Mentoring:

● Assign experienced nurses to the participants as preceptors and mentors.

These mentors will offer direction, support, and clinical insights.

● Encourage preceptors to promote a learning, cooperation, and open

communication culture throughout the program.

Clinical Rotations:

● Plan a variety of clinical rotations across departments to expose participants

to a variety of specialties and patient demographics.

● Rotate participants through high-demand units, which may result in increasing

interest in these areas.

Educational Workshops:

● Organize regular workshops led by both internal and external experts. These

workshops can cover topics like evidence-based practice, communication

skills, and dealing with complex cases.

9
Professional Development:

● Provide options for ongoing education, such as attending conferences, getting

certifications, and enrolling in advanced courses.

● Collaborate with educational institutions to provide tuition help or higher

degree scholarships.

Retention Strategies:

● Extend the program to existing nurses within the hospital who may be

considering leaving due to burnout or a lack of growth possibilities, in addition

to newly minted nurses.

● Customize the program to fit the specific needs of experienced nurses,

including professional progression opportunities.

Evaluation and Adaptation:

● Regularly assess the program's effectiveness through surveys, feedback

sessions, and performance evaluations.

● Make necessary adjustments based on feedback to ensure the program

remains relevant and impactful.

Benefits:

Attracting New Nurses: The program positions the hospital as a desirable

workplace for new graduates by offering structured support during their

transition to practice.

10
Retaining Experienced Nurses: The program provides a chance for

experienced nurses to rejuvenate their careers and explore new avenues

within the hospital.

Improved Patient Care: The comprehensive training and mentorship

provided by the program ensure that nurses are well-prepared to deliver

high-quality patient care.

Addressing Burnout: By providing a nurturing environment and opportunities

for growth, the program can help alleviate burnout among existing nurses.

Long-Term Solution: Graduates of the program would likely have a strong

affinity for the hospital, potentially leading to a higher retention rate.

11
CHAPTER 3
CONCLUSION

On the whole, nurse shortage is a burden of healthcare as a whole and critical or

intensive care units in particular. Still, the problem can be addressed in case the

organization is open to change. Thus, it is necessary to define strengths,

weaknesses, opportunities, and threats, both internal and external and address them

on an organizational level. The knowledge of weaknesses and threats as well as

attention to strengths and opportunities allow improving the situation with nurse

shortage through the creation of more favorable work conditions and stimulation of

professional development.

In conclusion, the case study of nurse shortage in the hospital highlights a critical

issue that has far-reaching implications for patient care, nurse well-being, and overall

healthcare system functionality. Throughout the analysis, several key points have

emerged, shedding light on the challenges posed by nurse understaffing and

suggesting potential solutions to address these challenges.

Firstly, the case study underscores the undeniable impact of nurses on patient

outcomes. Overworked and overwhelmed nurses face difficulties in providing timely

and high-quality care to patients. This can result in medical errors, delayed

treatment, and compromised patient safety. Consequently, hospitals with insufficient

nurse staffing may experience higher rates of patient complications and increased

hospital readmissions, thereby elevating healthcare costs and straining resources.

Moreover, the well-being of nurses themselves is significantly compromised under

such conditions. Chronic understaffing leads to increased stress, burnout, and job

12
dissatisfaction among nurses. As they struggle to manage heavy workloads, their

physical and mental health suffers, ultimately leading to higher turnover rates and a

shortage of experienced nurses in the long term. This cyclical pattern perpetuates

the problem and creates a vicious cycle that hampers the hospital's ability to provide

consistent and effective patient care.

The case study also highlights the broader systemic challenges that contribute to

nurse understaffing. Factors such as budget constraints, recruitment difficulties, and

fluctuating patient volumes can all contribute to the staffing crisis. Hospitals must

address these underlying issues to implement effective solutions that ensure

adequate nurse staffing levels.

Addressing the issue of nurse understaffing requires a multi-faceted approach.

Hospitals need to invest in comprehensive workforce planning, anticipating patient

needs and adjusting nurse staffing levels accordingly. This involves real-time

monitoring of patient admissions, discharges, and acuity to ensure an appropriate

nurse-to-patient ratio. Collaboration between hospital management, nursing

leadership, and human resources is essential to accurately forecast staffing needs

and make necessary adjustments.

Furthermore, hospitals should invest in strategies to retain their nursing staff. This

includes offering competitive compensation, providing opportunities for career

advancement, and creating a supportive work environment that values nurses'

contributions. Implementing flexible scheduling options, adequate breaks, and

support programs for stress management can significantly improve nurse morale and

job satisfaction.

13
In conclusion, the case study of understaffed nurses in the hospital underscores the

urgent need for a holistic approach to address this critical issue. By recognizing the

profound impact on patient care quality, nurse well-being, and the healthcare system

as a whole, hospitals can prioritize effective workforce planning, nurse retention

strategies, and collaborative efforts across departments. Ultimately, solving the

problem of nurse understaffing requires a commitment to providing optimal patient

care while also valuing and supporting the dedicated nurses who form the backbone

of the healthcare industry

14
RECOMMENDATION

Workforce Planning and Recruitment:

Develop a robust workforce planning strategy that aligns nurse staffing levels with

patient acuity and volume. This involves using data analytics to predict peak

admission periods and adjusting staffing accordingly.

Establish partnerships with nursing schools and educational institutions to create a

pipeline for new nursing graduates. Offer internship programs and scholarships to

attract potential candidates.

Competitive Compensation and Benefits:

Conduct a thorough analysis of nurse salaries in the region and ensure that your

hospital offers competitive compensation packages. This can include base pay, shift

differentials, overtime pay, and bonuses.

Provide comprehensive benefits such as health insurance, retirement plans, and

tuition reimbursement to attract and retain nurses in the long term.

Nurse Retention Strategies:

Implement a comprehensive orientation and onboarding process for new nurses to

ensure they feel supported and confident in their roles from the beginning.

15
Offer opportunities for career advancement, including pathways to specialty

certifications and leadership roles within the hospital.

Flexible Scheduling:

Introduce flexible scheduling options, such as self-scheduling and rotating shifts, to

accommodate the diverse needs of the nursing staff.

Implement technology-driven scheduling solutions that allow nurses to request shift

changes and time-off requests easily.

Support Programs and Well-being Initiatives:

Establish wellness programs that focus on stress management, mental health

support, and work-life balance. Provide access to counseling services and wellness

resources.

Create designated spaces for relaxation and recharge within the hospital for nurses

during breaks.

Technology Integration:

Invest in healthcare technology solutions like Electronic Health Records (EHR)

systems and nurse call systems to streamline workflows and reduce administrative

burdens on nurses.

16
Implement telehealth platforms to provide remote consultations, reducing the need

for nurses to be physically present for certain tasks.

Collaboration and Communication:

Foster a culture of open communication and collaboration between nursing staff,

nursing leadership, and hospital administration.

Conduct regular feedback sessions to address concerns and gather insights from

nurses about their needs and challenges.

Community Engagement:

Engage with the local community through awareness campaigns and recruitment

drives to attract nurses who may be interested in working closer to home.

Offer community-based training programs to upskill individuals interested in pursuing

a nursing career.

Data-Driven Decision-Making:

Continuously monitor and analyze nurse turnover rates, patient outcomes, and

staffing levels. Use this data to make informed decisions and refine your strategies.

17
Advocacy and Policy Initiatives:

Collaborate with professional nursing associations to advocate for policies that

support nurse staffing ratios and safe working conditions.

Engage with local and national policymakers to raise awareness about the critical

importance of addressing the nurse shortage.

By combining these recommendations, hospitals can create a comprehensive

strategy to alleviate the nurse shortage, enhance patient care, and create a more

supportive and fulfilling work environment for nursing staff.
