AFFIDAVIT OF UNDERTAKING
I, EMERSON GENESES J. BUHIA, of legal age, Filipino citizen, married and
a resident of Don Carlos, Bukidnon, after having been duly sworn to in accordance with
law do hereby depose and say:

1. That I am the School Principal of Lake View Adventist Academy, Don Carlos,
Bukidnon.

2. That we are presently processing the necessary permit of our school at the Local
Government Unit (LGU) Don Carlos, Bukidnon and one of the requirements is the
land title where the school is built.

3. That the institution is still on the process of titling the land and is waiting for
decision of the higher office/s regarding its granting.

4. That I hereby undertake to exempt and free from any liability or accountability
the LGU-Don Carlos or any of its representative to whatever outcome of the land
contest of the said institution, in consequence of the processing and/or granting
of our permit pending submission of the subject land title.

5. That I further undertake that LGU-Don Carlos will not be held liable to whatever
legal cases that may occur in the institution.

6. That I am executing this affidavit to attest to the veracity of the foregoing

statements and for whatever worth this may serve best.
