AFFIDAVIT OF UNDERTAKING
I, MHEGAN FAYE B. FELONGCO, of legal age, Filipino citizen, single and a
resident of Memorable Village, Poblacion, Pangantucan, Bukidnon, after having been
duly sworn to in accordance with law do hereby dispose and say:

1. That I am a student Central Mindanao University, Musuan, Maramag, Bukidnon

taking up Bachelor of Secondary Education Major in Sciences, and currently
enrolled in second semester school year 2023-2024.

2. That I would like to transfer at the University of Science and Technology of

Southern Philippines, Cagayan de Oro City.

3. That the said institution requires me to submit credentials from my previous

school as a requirement for my enrolment process, such as but not limited to
Transcript of Record (TOR);

4. That by virtue of this affidavit, I hereby undertake and bind myself to submit all
the credentials I am required to present within the month of August 2023.

5. That I am executing this affidavit to attest to the veracity of the foregoing

statements, to support my enrolment for academic year 2023-2024, and for
whatever worth this may serve best.
