Note:
• Alcohol use and gallstones are the most common causes of acute pancreatitis.
• Do not assign codes to symptoms (e.g., midabdominal pain) when a definitive diagnosis is
established. In this case, the patient was definitively diagnosed with acute pancreatitis and
cholecystitis.
• Do not assign a code to cholelithiasis because this condition was not evaluated during the
admission; it will be evaluated on an outpatient basis.

Note:
• Do not assign codes to symptoms such as fatigue, shortness of breath, and chest pain when
a definitive diagnosis has been established. In this case, the patient was diagnosed with
congestive heart failure.
• According to the AHA’s Coding Clinic, “Pleural effusion is commonly seen with congestive
heart failure . . . pleural effusion documented only as an x-ray finding without the physician
having made such a diagnosis should not be reported.” In this case, congestive heart failure
should be documented as the principal diagnosis.

Note:
Documentation indicates that the patient was administered insulin during the hospitalization,
and the patient is discharged with a prescription for insulin. Although the patient receives insulin
during the hospitalization and continues to receive insulin upon return to the nursing facility, a
physician query should be generated to ask if the patient’s diabetes is type 1 or 2.

Even though the radiologist documented “frontal sinusitis” on the radiology report, the attending
physician should be asked if she concurs that “frontal sinusitis” is the diagnosis that should
be coded. Official coding guidelines state that abnormal findings (e.g., x-ray results) are not
coded and reported unless the physician indicates their clinical significance. If the findings are
outside the normal range and the physician has ordered other tests to evaluate the condition
or prescribed treatment, it is appropriate to ask the physician whether the abnormal finding
should be added. Also do not assign codes to the headache and facial pain because they are
symptoms of the frontal sinusitis.
