For home monitoring, if parameters remain outside safe ranges for 5 minutes, an alert is sent to the nearest hospital and ambulance service.

At present in hospitals, based on patient health condition they need to
check regularly. If there is sudden change in their patient health condition, it is not immediately known to the healthcare professionals. So, to overcome that we are using health sensors and with the help of these sensors gradually changes in the patient health condition are known. A patient health monitoring system using IoT (Internet of Things) with RFID (Radio Frequency Identification) and GSM (Global System for Mobile communications) technology can provide real-time health data monitoring and communication capabilities in hospitals and homes. Various health monitoring devices equipped with sensors can be attached to the patient. These devices can include heart rate monitors, blood pressure monitors, pulse oximeters, temperature sensors, glucose meters, and more. Each doctors and nurse can be assigned an RFID tag, either as a wristband or a small tag attached to their clothing. The RFID tag will have a unique identifier associated with the profile. These devices will continuously collect health data and transmitted to receiver. If any critical health parameter falls outside the safe range, the system triggers real- time alerts through the GSM module to receiver of the control room. The alarm triggers until the system can be detect the assigned RFID tag and the data can be stored in cloud. The cloud or server receives and stores the patient's health data from the IoT gateway. In homies if any critical health falls outside the safe range, the triggers alarm. If alarm triggers continuously five minutes then the alert transmitted to the nearest hospital and ambulance service.
