allow or authorize him/her to undertake the On – The – Job Program as an academic
requirement for graduation. I further agree that the said program be conducted at Olalia Law
Office – City of Ilagan following the University rules on Student Conduct and that of the
agency.

I hereby further agree to shoulder all personal and incidental expenses that may be
incurred by him/her while undergoing the OJT Program and that the University and the host
agency will not be held responsible for any untoward incident pertaining thereto. It is
