Instructions

1. NAME OF FORM:
Application for Certification of Small Marine or Mature Marine Oil Fields

Definitions:
Crude oil field means:
- A single reservoir, or multiple reservoirs all grouped on, or related to, the same
geographical structure or stratigraphic conditions from which crude oil and
associated gas may be produced.

Small marine oil field means:

-A marine oil field which produces less than 1500 barrels of oil equivalent per day (boepd).

Mature Marine Oil Field means:

- A marine oil field that is twenty-five (25) years or older from the date of its first
commercial production.

2. LICENCE/CONTRACT:
Licence number or block under a production sharing contract.
Prepare separate submissions for each licence or contract.

3. OPERATOR:
State the name of the oil company operating the field.

4. OTHER PARTIES:
State names of joint partners and percentage ownership.
(No separate application is required from joint partners)

5. QUARTER ENDING:
State the calendar quarter being reported – e.g. 31st March, 30th June, etc.

6. SMALL MARINE/ MATURE MARINE:

Check appropriate box and prepare separate submissions for small marine or mature
marine fields.

PS: Small marine fields – applicable to columns 1, 3 & 4 only.

Mature marine – applicable to columns 1 & 2 only.

7. FIELD NAME:
State the name of the field for which production is being reported

8. DATE OF FIRST COMMERCIAL PRODUCTION:

State the date of first commercial production of crude oil from this field (regardless of
ownership of licence or contract at that date).

9. TOTAL PRODUCTION FOR THE QUARTER:
State the total production of the field for the quarter in barrels of oil equivalent (boe).

10. AVERAGE DAILY PRODUCTION FOR THE QUARTER:

State the average daily production for the quarter of each field in barrels of oil equivalent
per day (boepd).

11. DECLARATION:
 NAME: The name of the duly authorized officer of the company who verifies the
report as being true and correct.

 SIGNATURE: The duly authorized officer of the company shall sign the report as
being true and correct.

 TITLE: The job or position of the duly authorized officer of the company who
signed the form.

12. DATE:
