AFFIDAVIT OF CONFIRMATION FOR THE DISCREPANCY OF THE NAME OF


THE BUYER
  
            I, ISMER JOHN B. CABUSAO, a Filipino citizen, of legal age, and with
residence at Poblacion, Bilar, Bohol, Philippines after having been duly sworn
in accordance with law, hereby depose and say:

1. That I am confirming the buyer in the Affidavit of Waiver of rights


of a parcel of land with Lot No. 4008-G-6-B of the subdivision plan,
PSD-____ being a portion of Lot No. 4008-G-6, PSD-07-104213,
situated at District of Tiptip, City of Tagbilaran, Province of Bohol,
Island of Bohol containing an area of Three Hundred Fifty-Six (356
sq. m.) square meters more or less, bearing Doc.No.42, Page No. 9,
Book No. IX, Series of 2023 executed on May 08, 2023 before
Notary Public Loreto B. Doria is ISMER JOHN B. CABUSAO.

2. That I hereby confirm that the buyer was inadvertently stated as


ISMER JHON B. CABUSAO instead of the correct one which is
ISMER JOHN B. CABUSAO.

3. That as such I respectfully respect that the correct entry that


should be reflected in the records shall be ISMER JOHN B.
CABUSAO.

4. That I am executing this affidavit to attest to the truth of the


foregoing facts and allegations;

5. Further Affiant sayeth none
