Order number _041046 dated 5-04-2021 of this office, is resorted to because it is critically
needed. State in narrative the circumstances that generated the urgency) and that:

a. The item is/are not available in our stock not the service can be provided by or
narrative unit.
b. There is no splitting of Procurement Order.
c. There is no material to procure/test requirements through the normal bidding
process.
d. The price/s are the lowest obtainable as a result of the canvass from the following
