NATIONAL REFERENCE LABORATORY FOR CLINICAL CHEMISTRY

ANNOUNCEMENT:

March 28, 2016

2016 National External Quality Assessment Scheme for Clinical Chemistry (NEQAS-CC).

Requirements for Registration to LCP-NRL (DEADLINE: MAY 30, 2016)

1. Original copy of fully accomplished Registration Form 302 and Form 303
2. Original copy of Order of Payment Form 304 and 305
3. Photocopy of your latest License to Operate (LTO)
4. Machine validated Transaction (Deposit) Slip - if payment is made via Bank to Bank Transaction

- Incomplete requirements shall not be processed.

- Emailed or faxed Registration Forms are considered unofficial and
shall likewise not be processed.

The 2016 NEQAS-CC year cycle participation fee for the below 10 analytes is P 8, 000.00
(Department Memorandum No. 2009-0086-B).

Analytes to be tested (choice of 1 or more from among the following):

Glucose Creatinine Cholesterol Albumin Potassium

Blood Urea Nitrogen Uric Acid Total Protein Sodium Chloride

Please take note of the following:

1. EACH ADDITIONAL ANALYTES will be charged a separate fee of P 8,000.00

2. Payment may be paid in cash or check (payable to LUNG CENTER OF THE
PHILIPPINES).
3. Personal check is NOT acceptable.
4. Certificate of Participation (COP) issued by the LCP-NRL-CC will ONLY be given to the
laboratory participants who have completed (all 12 samples and PAID all unsettled
accounts) the year cycle.
This COP is a documentary requirement for the renewal of your License to Operate.
