AFFIDAVIT

I, ROSELLE ATALIP CORPUZ, of legal age, Filipino, single and a resident of

Zone 2 Taytay, Baggao, Cagayan, after having been sworn in accordance with law, do
hereby depose and state that:

1. On May 3, 2023, I filed a Police Blotter at Baggao Police Station due to the attacks
made by my relative against me and my companions ODETTE VALLO and
KAREN GALLARDO;

2. On the same date, one of my companions KAREN GALLARDO suffered

abrasion on the right part of her face caused by the attacks and was examined at
Baggao District Hospital;

3. In the morning after the incident, I noticed that I also suffered abrasion in my
right arm caused by the attacks. After which, I immediately went to Baggao
District Hospital for examination;

4. On May 5, 2023, A Medico – Legal Examination Certificate was issued to me by

Dr. Beverly Anne B. Ibay. As evidence, the certificate is herein attached;

5. I am executing this affidavit to the truthfulness of the foregoing and for whatever
legal purpose it may serve.
