AFFIDAVIT OF DISCREPANCY

I, , of legal age, married, and a resident of Barangay San Pablo, Tarlac City,
after having been duly sworn to in accordance with law, do hereby depose and
state THAT:

1. I am the registered owner of a parcel of land situated in Barangay San

Sotero, Sta. Ignacia, Tarlac and covered by Transfer Certificate of Title
(TCT) No., of the Registry of Deeds for the Province of Tarlac;

2. In the Tax Declaration and Certificate of No Improvement of the subject

property, my middle initial was erroneously entered as “G” instead of “C”
as correctly appearing in TCT no. and my Driver’s License;

3. My true and correct middle initial is “C” and not “G”,

4. I am executing this affidavit to attest to the truth of the foregoing facts and
for whatever legal purposes it may serve.
