3. Family details issued by employer
4. Composite claim form
5. Nominee - PAN/Aadhar/Bank cancel cheque 
6. Member - PAN/Aadhar/Death certificate
7. Passport size photograph nominee
8. Last three ECR copy - PF
9. Marriage certificate if married
10. If kids four combine colour photos (Nominee & kids).
11. Four passport colour photos of nominees and (if kids). 
12. Bank A/c in same bank but separate with each other (in case kids are there) 
13. Death certificate.
14. Affidavit for declaration availing the benefits (nominee & kids). 
15. Contact details of nominee
16. one cancelled cheque of each family member.
