Cenita DOS WITH ASSUMPTION OF MORTGAGE - PAG-IBIG
This Deed of Sale with Assumption of Mortgage made and executed by and between:
NELNA PERALTA SARABIA, Filipino, of legal age, single and residing at 58 Zorra St.,
Paltok, Quezon City, NCR-Second District, herein referred to as the VENDEE,
WITNESSETH:
WHEREAS, the VENDOR is the present owner of a parcel of land located at Pinoy
Village, Barangay Sabutan, Silang Cavite with an area of FORTY (40) SQUARE METERS
covered by Transfer Certificate of Title No. 076-2020000402 including the residential house
found thereon by virtue of Contract-To-Sell executed on October 15, 2020 docketed as Doc No.
316; Page No. 64; Book No. 196; Series of 2020 under the notarial register of Atty. Joshua P.
Lapuz. The said parcel of land is more particularly described as follows:
WHEREAS, the VENDEE is fully aware that the subject properties are still being
amortized at the time of this Deed of Sale and the VENDEE shall assume the total and
outstanding obligation of the VENDOR to PAG-IBIG Home Development Mutual Fund.
Whereas, VENDEE hereby binds herself to pay monthly amortization of the above-mentioned
mortgaged property until full payment;
WHEREAS upon full payment by the VENDEE of the monthly amortizations due, the
VENDOR shall execute the Deed of Absolute Sale over the property and surrender
possession and control thereof to the VENDEE;
That the VENDOR join the VENDEE in requesting the authorities for the approval,
recognition, institution of the VENDEE, the claims, rights, interests and ownership over the
above-mentioned house and lot and obliges himself to execute, sign and deliver any and all
documents or papers which may be necessary for this purpose.

License ID No. D18-12-011087 issued by LTO, valid until 2023/11/2; ELVIE MERCADO
CENITA with her Voter’s ID No. 2118-0066A-K1787EMC20000 issued by Comelec Silang,
Cavite and NELNA PERALTA SARABIA with her Voter’s ID No. 7404-0642A-C2590NPS20000
issued by Comelec Quezon City, NCR-Second District, presenting an integrally complete
instrument or document entitled Deed of Sale with Assumption of Mortgage consisting of two (2)
pages including this page in which this acknowledgment is written, is attested to be personally
known to me or identified by me through the above-indicated competent evidence of identity as
defined by the Rules on Notarial Practice of 2004, as the same persons who signed the above-
mentioned instrument in my presence and they represented to me that the signatures on the
instrument or document were voluntarily affixed by them for the purposes stated in the
instrument or document as their free and voluntary act and deed. WITNESS MY HAND AND
SEAL ON THE PLACE AND DATE FIRST ABOVE-WRITTEN.
“To appear and represent me in any and all transactions before the Pag-
IBIG FUND Home Development Mutual Fund concerning my housing loan application
located at Pinoy Village, Barangay Sabutan, Silang Cavite with an area of FORTY (40)
SQUARE METERS covered by Transfer Certificate of Title No. 076-2020000402.”
“To represent and transact official business with the Office of the Registry
of Deeds, Assessor’s Office, Bureau of Internal Revenue and other agencies in
connection with the cancellation of mortgage and transfer of ownership in her name; to
execute, sign and deliver Promissory Note/Notes in favor of the Pag-IBIG FUND, as
well as other documents that may be required by the said government financial
institution in connection with the said housing loan;
“To make, sign and execute, for and in my behalf, any documents which
may be necessary for the release of the Certificate of Title;
HEREBY GIVING AND GRANTING unto my said attorney-in-fact full powers and
authority to do and perform all and every act and thing whatsoever requisite and
necessary to be done in and about the premises, as fully to all intents and purposes, as I
might or could do if personally present, with full power of substitution and revocation and
HEREBY RATIFYING AND CONFIRMING all that our attorney-in-fact shall lawfully do or
cause to be done by virtue hereof in pursuance of the purposes above-stated.
