HOW TO TREAT PROPER

Wound Care
01 ASSESS THE
PATIEN'TS WOUND
Assess the appearance of the wound the
color, size, location and what type of
wound. If necessary, measure using a
measuring guide or used ruler to measure
the length, width and depth and look if
there is presence of odor in the wound.

CLEANSE THE
02 WOUND OF THE
PATIENT

Clean the area of the wound from the


center of the wound outward using
circular strokes. Use a separate swap
for each stroke.

IRRIGATE THE
03 WOUND USING A
SOLUTION

Instill a steady stream of irrigating


solution into the wound. Make sure
all areas of the wounds are irrigated.
Continue to irrigate until the solution
becomes clear

ASSESS FOR ANY


04 SIGNS OF PARTICULAR
TYPES OF EXUDATE

Exudate is produced from fluid that has


leaked out of blood vessels and closely
resembles blood plasma. Assess the
appearance of the wound again, noting in
particular the type and amount of exudate
still present and the presence and extent of
granulation tissue.

DRESS THE
05 WOUND OF THE
PATIENT
Using sterile technique,
apply a dressing to the
wound based on the amount
of drainage expected.

PATIENT HEALING
06 PROCESS

Continue this care until


healed.
