Nurse: Good morning, Ms. Cara. The doctor will be in shortly.

And I need to take some


questions about your past medical history, if that’s OK.
Patient: Of course. What do you need to know?
Nurse: Well. Your medical form says you’re suffering from headache.
Patient: Yes. I’ve had this problem on and off for many years. I’ve been worried because
it’s getting worse every time.
Nurse: When did you have this problem for the first time?
Patient: It was 3 years ago, I think so.
Nurse: Have you seen a doctor for the condition?
Patient: No. I just take some aspirin everytime I have headache.
Nurse: Do you usually smoke or drink alcohol?
Patient: Absolutely not. I don’t like either of them.
Nurse: OK. Does anyone in your family have a history of brain problems?
Patient: No, not that I know of. Why would you want to know that?
Nurse: Because we have to get as much information as we can for the doctor so he can
make an informed diagnosis about your condition and it’s treatments. The doctor will be in
to see you soon. Thanks for your time.
Patient: OK, you too.
