AFFIDAVIT OF CONSENT, WAIVER AND TRANSFER

I, BENITA M. BINARAO, of legal age, Filipino, married, and a resident of

Boloc-boloc, Sibulan, Negros Oriental, after having been duly sworn to in accordance
with law, state and declare that:

1. I have an account with Negros Oriental II Electric Cooperative (NORECO

II), with Account Number: 0303330610, Meter No.: 2006647 in B6 L16 Estanzia
Subdivision, Maslog, Sibulan, Negros Oriental;

2. I hereby give my consent to TRANSFER and WAIVE unto and in favor

of, ANNABELLE C. BINARAO, of legal age, married, Filipino and a resident of
Maslog, Sibulan, Negros Oriental, all of my rights of ownership, interest and/or
participation in and over my above-mentioned NORECO II account ownership or
membership covering or involving the electrical connections of said house or building,
absolutely and unconditionally and free from any lien or encumbrance.

3. I further undertake that the NORECO II approving the said application is

not to be made accountable relative to the said application and waiver and we take full
responsibility in case of any claims or complaints thereon;

4. I am executing this affidavit to establish all of the foregoing facts.
