AFFIDAVIT OF NON-OPERATION

I, Heddo Lindo A. Sanchez, of legal age, married, Filipino and with residence address at
Blk.13 Lot 7 Gallera de Oro Subd. Bago Aplaya Talomo Davao City, after having been duly
sworn to in accordance with law, hereby depose and state:

1. That I am the President of Davao Horizon PC Professionals Inc. with business

address at Unit 5-122 1st Flr. Bldg.5 One Oasis Ecowest Drive, Ecoland Brgy.Bucana,
Davao City;
2. That the aforesaid corporation has been registered in Security of Exchange
Commission on November 27, 2017 but it was not yet operated till December 2017.
That for this reason there was no income from the said business;
3. That I am executing this affidavit to attest to the truth of the foregoing statement to
inform the proper authorities concerned and for whatever legal purpose this may
serve.
