AFFIDAVIT OF LOSS

I, HERMINIA B. TORRE, of legal age, Filipino, widow and a resident of Barangay

9, Bacacay, Albay after being sworn in accordance with law, hereby depose and state
THAT:
1. I am in custody and in possession of the Transfer Certificate of Title (TCT)
No. PA-3566 of a parcel of land, owned and registered in the name of the
deceased LOURDES TORRE. I placed said TCT in an envelope which I
customarily kept inside the cabinet in my room in our house located at
Barangay 9, Bacacay, Albay;
2. That however in November, 2020, during the onslaught of the super
Typhoons “Rolly” and “Ulysis” said TCT was among the records together with
my other personal belongings unfortunately destroyed due to heavy rain and
flooding brought about by the said Typhoons;
3. That by reason of the above fact and circumstance, and considering that said
Title is beyond recovery, it is already considered lost, by all purpose and
intent;
4. I certify and confirm that the parcel of land covered by Transfer Certificate of
Title No. PA-3566 of the Registry of Deeds for the Province of Albay was
never mortgaged, sold, conveyed, disposed of, or encumbered, to any other
person or entity. I certify and confirm further that said document was neither
confiscated nor relinquished in favor of, any government entity agency, or
authority; 
5. This affidavit is being executed to attest to the truthfulness of the above facts
and to support the petition before the Regional Trial Court for the issuance of
an Order directing the Register of Deeds of Legazpi City to issue replacement
of the lost OWNER’S COPY with the same force and effect as the lost one.
