Person with Disability (PWD) and is unable to travel independently due to her medical condition. I
trust her completely and believe that she will handle all necessary tasks with utmost care and
responsibility.

The specific authority granted to Marinel De Jesus Aguilar includes, but is not limited to:

1. Representing me in legal matters, including but not limited to signing legal documents,
contracts, or agreements on my behalf.
2. Assisting in the preparation, submission, and collection of legal paperwork, such as
applications, forms, or claims.
3. Acting as my representative in dealings with government agencies, legal entities, or
organizations.
4. Receiving and reviewing legal correspondence, such as letters, notices, or documents, and
providing necessary responses or actions as required.
5. Accessing and obtaining my medical records, including test results, treatment plans, and
other medical information, from healthcare providers or medical institutions.
6. Making appointments with healthcare professionals or specialists on my behalf and
accompanying me to medical appointments when necessary.
7. Discussing medical diagnoses, treatment options, and other healthcare-related matters with
healthcare professionals on my behalf.

This authorization is valid from [July 10, 2023] to [October 10,2023], during which Marinel De Jesus
Aguilar has the full authority to act on my behalf in all matters related to legal paperwork and
medical affairs.

Please note that this authorization letter should be presented to any relevant parties or institutions
to confirm Marinel De Jesus Aguilar's authority to act on my behalf. I will not hold you responsible for
any actions taken in good faith by Marinel De Jesus Aguilar in accordance with this authorization.
