Dear Parent/Guardian,

You child has expressed an interest in volunteering to work with Mr. Pora and Mr. Powell on
Friday May 5th from 11am-2:30pm. They will be working with students and supporting them in
class. The students must be promptly picked up after the school day concludes at 2:45pm. We
appreciate the efforts of your child and look forward to working with them on Friday.

Please sign the permission slip below and return the permission form upon entering the school.
Your child will need to sign in as a guest in the office and hand in the signed permission form at
that time. Thank you again for all of your support.
