OB WARD TIMELINE OF ACTIVITIES

DAY 2 CLIENT’ S POSTPARTUM DATA
(8:00–4:00pm)  You receive a Primi patient post NSD from D.R. with ongoing IVF of D5LR 1L
at the level of 640cc, regulated at 25gtts per min. hooked at the right hand and
infusing well.
 Upon receiving, you are going to have a quick check on the following:
- General appearance of the mother (findings should be healthy)
- Check the fundus (approximately one hour post-delivery, the fundus is
firm and at the level of the umbilicus. The fundus continues to descend into
the pelvis at the rate of 1 cm or fingerbreadth per day and should be non-
palpable by 14 days (about 2 weeks) postpartum.
- Assess the breasts for signs of engorgement, including fullness.
- Assess vaginal discharge.
- Vital Signs checked upon admission at 8am then every 2hours.

Documentation/Charting (SOPIE)
 Patient Carol is feeling well except for the pain she felt in her perineum due to
episioraphy
 Patient Carol is taking oral medication (mefenamic 500 mg 3x a day,
amoxicillin500 mg 3x a day, ferrous sulfate 325 mg 3x a day and ascorbic
acid 500 mg once a day)
 She is advised by the physician to do breastfeeding, but she has difficulty
doing it since this is her first time to have a baby and claiming there is no
breast milk that comes out and the baby looks uncomfortable when she carries
and try to nudge on her breast.
 During interview, you asked the patient to rate the pain she felt in her
perineum using the pain scale of 1 – 10 (1 as the lowest and 10 as the highest
rate) the patient verbalizes 7/10 and she is not comfortable about it and feels
afraid to move. During the last part of your interview, she verbalizes that she
wants to know whether she should start using contraception now, as she does
not want to become pregnant again for at least 2 years.
 All other aspects of her history are normal
 Identify 2 priority Nx Dx (to be mentioned towards the end of your video)
 All output shall be submitted until 11:55pm on same day,

DAY 3  Giving of Instructions to students regarding their 2nd Week activities:
(8:00–4:00pm)  OB Ward Setting – After 24 hours with Patient at the Ward
 Total Patient Care
- Students will look for 2 trusted individuals to act as a patient and as a
colleague
- Endorsement (student will receive the patient from the outgoing
nurse).
- V/S taking and recording
- Administration of oral medication with teachings
- Charting: SOPIE
 After the meeting – students will simulate the given scenario

DAY 4  Students' preparation


(8:00–4:00pm)  Simulation of Scenario
 Completion and Submission of Requirements for 1st week

 Students are to simulate the following: OB Ward Setting – Postpartum


Assessment &Care
As the nurse assigned to the Patient You are expected to do the following:
 Receive the patient from the outgoing nurse
 Check and record vital signs
 Administer oral medication
 Do nurse-patient interaction:
 You noticed that during your interaction, the patient did not yet change
her clothes and did not take a bath yet.
 Patient is feeling well but has sore nipples. During observation of
breastfeeding, it was found that the baby was not latching well to the
breast.
 Patient report that the baby breastfeeds approximately every 2 hours but
now that her nipples are sore, she is afraid to breastfeed her baby
regularly.
 You delivered health teachings on proper hygiene (but patient verbalizes that
she was instructed by her mother not to take a bath at least for 3days to avoid
having post-partum complications.
 Documentation/Charting (SOPIE)
 Identify 2 priority Nx Dx and formulate Nursing Care Plan

DAY 5  Giving of instructions for the 3rd week’s activities
(8:00–4:00pm)  You are going to simulate the following:
 Your patient is for discharge.
 As the nurse assigned to Patient Carol you are in charge of discharging her.
 Before the patient leaves the ward, you are going to give your discharge
plan and teaching to patient which includes the following:
 Diet
 Medication
 Proper Hygiene
 Breastfeeding
 Follow up check up
 Newborn Care at home
 Student’s Preparation

DAY 6  Students monitoring


(8:00–4:00pm)  Students will simulate and record their output
 Students’ submission of output

EXPECTED OUTPUT
 Record of Simulated Scenario (discharge plan & teaching)

DAY 7  Based on the scenario, you are going to identify a health education topic and
(8:00–4:00pm) create an infomercial.
 Time frame for recording - (5-10mins).

DAY 8  Submission of output
