JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

WE, ANTONIO DIAZ ESGANA and REMINIA NATI GARCIA, both of legal age,
Filipinos, and both residents of Sampaloc, Dasmarinas City, Cavite, after having been duly
sworn to in accordance with law hereby deposes and say:

That we personally know the late WILFREDO GUIRUELA SISNO, who passed way on
April 08, 2023;

That he was the son of CRISPOLO SISNO and EUGENIA GUIRUELA;

That in his Certificate of Death, his parent’s name appears as “CRISPOLO SISNO and
EUGENIA GUIRUELA” but when his family’s about to claim his benefits in Social
Security System (SSS), they found out that his parent’s name was erroneously written as
“CHRISPOLO SISNO and EUGEN GUIRUELA”

That we hereby attest and testify that the correct and true name of his parents are
“CRISPOLO SISNO and EUGENIA GUIRUELA”;

That his parents, CRISPOLO SISNO and EUGENIA GUIRUELA and CHRISPOLO
SISNO and EUGEN GUIRUELA, as described herein, are refers to one and the same
persons;

That GENELYN BELMONTE CORNELIO who was born on April 04, 1988 and
DUMA-UP GENELYN CORNELIO who was born on April 09, 1988, as described
herein, are refers to one and the same persons;

IN WITNESS WHEREOF, we have hereunto set our hands this 27th day of July 2023,
