Complex Regional Pain Syndrome Involved In Upper Limp: Hand, Shoulder and Elbow
ABSTRACT
Complex Regional Pain Syndrome (CRPS) is a poorly understood chronic pain condition of
multifactorial origin. Complex regional pain syndrome (CRPS) is characterized by pain in a
limb in association with sensory, vasomotor, sudomotor, motor and dystrophic changes. Pain
is typically leading symptom. The diagnosis is clinical and one of exclusion. CRPS type 1 is a
disabling problem after stroke which effects the rehabilition goals: we should do early
mobilization to prevent CRPS but if CRPS is exist, we must relieve the pain. CRPS
frequently reported in plegic upper limb, also known as shoulder-hand syndrome. However,
only a small number of patients have CRPS-1 in the lower limb. In this article a 63 year-old
right hemiplegic woman who had ischemic stroke a year ago, complained of pain in
extremities, diagnosed as CRPS-1 in hemiplegic upper extremities: right hand, elbow and
shoulder. After medical and physical therapy her pain was relieved and it affected
rehabilitation process positively.

ÖZET
Kompleks Bölgesel Ağrı Sendromu (KBAS ), multifaktöriyel sebebi olabilen , sebebi tam
olarak açıklanamayan kronik bir ağrı durumudur. Kompleks bölgesel ağrı sendromu (KBAS),
duyusal, vazomotor, sudomotor, motor ve distrofik değişikliklerle birlikte bir uzuvda ağrı ile
karakterizedir. Ağrı tipik olarak önde gelen semptomdur. CRPS tip 1 inme sonrası sakat
bırakan bir sorundur ve rehabilitasyon hedeflerini etkiler: CRPS'yi önlemek için erken
mobilizasyon yapmalıyız, ancak CRPS gelişmişse ağrıyı hafifletmeliyiz. El omuz sendromu
olarak da bilinen CRPS sıklıkla plejik üst ekstremitede rapor edilmiştir. Bununla birlikte,
sadece az sayıda hasta alt ekstremitede CRPS-1'e sahiptir. Bu yazıda, bir yıl önce iskemik
inme geçiren 63 yaşında sağ hemiplejik bir kadın hastada , sağ üste ekstremide el, dirsek ve
omuzda ağrı şikayeti ile CRPS-1 tanısı alması incelenmiştir.. Medikal ve fizik tedavi
sonrasında ağrıları hafifledi ve rehabilitasyon sürecini olumlu etkiledi.

CRPS is a chronic pain condition that usually arises after a traumatic event
of the soft tissues. The “definitive” cause still remains unknown, although
different pathogenetic concepts have been proposed; three of the most
studied are: autonomic dysfunction, neurogenic inflammation, and changes
in CNS neuroplasticity, all of which are still in dispute. However, current
evidence shows that this problem could have a multifactorial origin.(2)
CRPS can be further divided into two subtypes, based on the absence
(CRPS I, previously known as reflex sympathetic dystrophy) or
presence (CRPS II, previously known as causalgia) of a major nerve
injury. It is distinct from other pain syndromes by the presence of
autonomic dysfunction, persistent regional inflammatory changes, and
a lack of dermatomal distribution. (3)
In this case report a 63 year-old right hemiplegic woman who developed CRPS-1 in right
upper limb is presented and discussed with the review of literature.
CASE REPORT
A 63 year-old female patient with right hemiplegia referred to our inpatient clinic for rehabilitation after
ischemic stroke. The
patient had a history of ischemic stroke a year ago and that
was the second time for her rehabilitation program. Her medical history included
hypertension and diabetes mellitus. Motor aphasic patient had a complaint of severe pain of right upper
extremities during her physical examination. The motor recovery stage of the right upper was 1 and right
lower limbs was 3 according to Brunnstrom motor recovery scale.
She did not have spasticity. On locomotor system examination; her right shoulder, elbow, dorsum of right
hand, right ankle was edematous and pale in colour, passive range of motion (PROM) of right shoulder
was restricted and passive movements of right shoulder, wrist, elbow and fingers were painful. PROM of
right ankle was painful.
The IASP proposed dividing the classification into types I and II, with type II being characterized also by
injury to a peripheral nerve., CRPS definition based on four criteria.(5)
The patient met the following criterias for right shoulder, elbow, hand: She had contiuninig pain with
hiperestethesia and allodynia in edematous and pale right upper extremity.
In radiologic examination; patchy osteopenia of right shoulder, elbow, hand and ankle with CRPS were
present. In three-phased bone scintigraphy revealed CRPS involvement of both right upper and lower
limbs (Figure 1). With the clinical and scintigraphic f indings the patient was diagnosed as CRPS-1 in the
right shoulder, elbow and hand; there is a increased activity involvement in 3,11,12 th ribs ,
trauma/fracture was considered primarily. Then she had a right ankle CT for differential diagnose trauma
or CRPS. CT showed that a fracture line on the right wrist.
Her diagnosis of CRPS is based on clinical findings. Imaging techniques can support diagnose and exclude
other possible reasons of symptoms. Osteopenia, soft tissue swelling or cortical resorption can be
determined in radiographic images. In a meta-analysis related to subject, reported that three-phase bone
scintigraphy is a sensitive procedure for the early diagnose.(6) Our patient had patchy osteoporosis in
both upper and lower extremities and three-phase bone scintigraphy supported the diagnosis of CRPS in
upper extremity.
CRPS-1 is a painful problem in stroke patients which effects patients to participation in rehabilitation
program. Early diagnose and treatment affect patient’s life quality, psychology and exercise participation.
In treating CRPS, one follows the classic order of rehabilitation beginning with pain and edema control,
followed by range of motion, then strengthening, and then function. It is important to convey to the
patient that immobilization is not an effective treatment for the pain and swelling; in fact, it may be
instrumental in the pathogenesis and chronicity of the process.(5)
Patient’s motor recovery stages were 1 in upper extremity and 3 in lower extremity. according to
brunnstrom motor recovery scale. She who was previously taken into a rehabilitaion program for
mobilization 2 months after ischemic stroke attack remained immobile after discharge. She had a shoulder
strap to prevent subluxation and she was able to walk with assisted. We believed that immobilization
played role in the development of CRPS-1 in our patient.
Her diagnosis of CRPS is based on clinical findings and supported by imaging techniques.
In CRPS-1 multiple treatment modalities have been described. In our patient, we used transcutaneous
electrical nerve stimulation (TENS), hotpack, therapeutic ultrasound and pregabalin to relieve pain. The
addition of TENS to the physical therapy program was seen to make a significant contribution to clinical
recovery in CRPS Type 1.(7)
In conclusion, in stroke patients with pain in paretic extremities CRPS-1 involvement of limb should be
kept in mind; there may be an unusual involvement such as elbow.

'Uluslararası Ağrı Çalışmaları Derneği, CRPS'yi, travma veya başka bir lezyondan sonraki
olağan ağrı seyriyle zaman veya derece olarak görünüşte orantısız görünen, devam eden
(spontan ve/veya uyarılmış) bölgesel ağrı ile karakterize bir sendrom olarak tanımlar. Ağrı
bölgeseldir (spesifik bir sinir bölgesi veya dermatomda değil) ve genellikle anormal duyusal,
motor, sudomotor, vazomotor ödem ve/veya trofik bulguların distal baskınlığına sahiptir. (1)

Bu olgu sunumunda sağ üst ekstremitede CRPS-1 gelişen 63 yaşındaki sağ hemiplejik kadın hasta
VAKA SUNUMU
63 yaşında sağ hemiplejili kadın hasta iskemik inme sonrası rehabilitasyon için kliniğimize başvurdu.
Hastanın bir yıl önce iskemik inme öyküsü vardı ve bu, 2. rehabilitasyon programı idi. Özgeçmişinde
hipertansiyon ve diabetes mellitus vardı. Motor afazik hasta fizik muayenesi sırasında sağ üst
ekstremitede şiddetli ağrı yakınması vardı. Brunnstrom motor iyileşme ölçeğine göre sağ üst
ekstremitenin motor iyileşme aşaması 1 ve sağ alt ekstremitenin motor iyileşme aşaması 3 idi.
Spastisitesi yoktu. Lokomotor sistem muayenesinde; sağ omzu, dirseği, sağ el sırtı, sağ ayak bileği
ödemli ve soluk renkli, sağ omuz pasif hareket açıklığı kısıtlı ve sağ omuz, bilek, dirsek ve
IASP, sınıflandırmayı tip I ve II olarak ayırmayı önerdi; tip II, aynı zamanda bir periferik sinir
Hasta sağ omuz, dirsek, el için aşağıdaki kriterleri karşıladı: Sağ üst ekstremitede ödemli ve soluk
Radyolojik incelemede; CRPS ile birlikte sağ omuz, dirsek, el ve ayak bileğinde yamalı osteopeni
mevcuttu. Üç fazlı kemik sintigrafisinde hem sağ üst hem de alt ekstremitede KBAS tutulumu görüldü
(Şekil 1). Klinik ve sintigrafik bulgularla hastaya sağ omuz, dirsek ve elde CRPS-1; 3,11,12.
kaburgalarda artmış aktivite tutulumu var, travma/kırık öncelikli olarak düşünüldü. Sonra ayırıcı tanı
travması veya CRPS için sağ ayak bileği BT'si tarafımızca çekildi. Tomografide sağ el bileğinde kırık
hattı görüldü.
CRPS tanısı klinik bulgulara dayanmaktadır. Görüntüleme teknikleri, semptomların diğer olası
doku şişmesi veya kortikal rezorpsiyon saptanabilir. Konuyla ilgili bir meta-analizde, üç fazlı kemik
sintigrafisinin erken tanı için duyarlı bir prosedür olduğu bildirilmiştir.(6) Hastamızın hem üst hem de
alt ekstremitelerinde yamalı osteoporoz mevcuttu ve üç fazlı kemik sintigrafisi KBAS tanısını üst
Etkilenen uzuvlara yani sağ üst ve alt ekstremitelere günlük hotpack, terapötik ultrason, transkütanöz
elektriksel sinir stimülasyonu (TENS) uygulaması ve pregabalin dozu kademeli olarak 300 mg/gün'e
CRPS-1 inme hastalarında ağrılı bir sendromdur ve hastaların rehabilitasyon programına katılımını
etkiler. Erken tanı ve tedavi, hastanın yaşam kalitesini, psikolojisini ve egzersiz katılımını etkiler.
CRPS tedavisinde, ağrı ve ödem kontrolü ile başlayan, hareket açıklığı, ardından güçlendirme ve
işlevsellik kazandırma ile başlayan klasik rehabilitasyon sırası izlenir. İmmobilizasyonun ağrı ve şişlik
için etkili bir tedavi olmadığını hastaya iletmek önemlidir; immobolizasyon KBAS sürecinin
Hastanın motor iyileşme evreleri brunnstrom motor iyileşme ölçeğine göre üst ekstremitede 1, alt
ekstremitede 3 idi. Daha önce iskemik inme atağından 2 ay sonra mobilizasyon amaçlı rehabilitasyon
programına alınan hasta taburcu olduktan sonra immobil kalmış. Subluksasyonu önlemek için omuz
askısı verilmiş ve yardıma ambule olabiliyordu. Hastamızda CRPS-1 gelişiminde immobilizasyonun
CRPS-1'de çoklu tedavi modaliteleri tarif edilmiştir. Hastamızda ağrıyı gidermek için transkutanöz
elektriksel sinir stimülasyonu (TENS), hotpack, terapötik ultrason ve pregabalin kullandık. TENS'in
fizik tedavi programına eklenmesinin CRPS Tip 1'de klinik iyileşmeye önemli katkı sağladığı görüldü.

Pregabalin, nöropatik ağrı durumlarında nispeten iyi sonuçları nedeniyle CRPS için kullanılır.
Pregabalinin nöronal aşırı uyarılabilirliği azalttığı ve artan GABA sentezi ile voltaja bağlı kalsiyum
kanallarının α2-δ alt birimine bağlanma arasında karmaşık bir sinerjiye sahip olduğu düşünülmektedir.
İkinci etki, glutamat, norepinefrin ve P maddesi gibi birçok nörotransmiterin salınımını azaltır.(8) İlk
seçenek olarak pregabalini tercih ettik ve hasta tarafından iyi tolere edildi. Doz artırımı ile hastanın
