“Paltos”

1st Handout:

ID: AI, 5 y/o, male and of Malinao, Albay

Informant: Mother (Percentile reliability: 100%)

CC: Itchy rash on the abdomen

HPI:

3 days PTC, AI had colds and dry cough and given Lagundi syrup at 5 ml 3x a day.

2 days PTC, he developed fever at 37.8C. Paracetamol 250 mg/5 ml at 5 ml was given every 4
hours as the fever was intermittent.
1 day PTC, fluid filled blisters appeared on the skin at the back and chest. The blisters feel itchy
so AI was given Cetirizine syrup at 5 ml at bed time.

ODC, AI still with intermittent fever and the cough becomes productive. He developed poor
appetite. The blisters increased in numbers and spread to the arms, abdomen and face. Hence the
aconsult.
