Introduction

Proton-pump inhibitors (PPIs) represent a class of drugs most prominently


known for their use in acid-related disorders.

They are the most potent suppressors of gastric acid secretion and can
diminish daily production of acid by 80-90 percent. 

PPIs are the prodrugs that get activated in acidic environment.

The proton pump inhibitor, omeprazole as pro-­‐drug is converted to its active


form in acidic medium. As weak base it specially concentrates in the parietal
cell, where it is activated by a proton-­‐catalyzed process to generate a
sulfenamide intermediate. The sulfenamide interacts covalently with
sulphydryl groups of cysteine residues in the extracellular domain of the
proton pump (H + K + -­‐ATPase) there by inhibiting its activity

lining of the fundus and in the body of the stomach. They secrete hydrochloric
acid and intrinsic factor"

Proton Pump

A proton pump is an integral membrane protein pump that builds up a proton gradient across
a biological membrane. Proton pumps catalyze the following reaction:

H+ [on one side of a biological membrane] + energy ⇌ H+ [on the other side of the membrane]

The activated drug molecules bind covalently to the sulfhydryl group of cysteines (amino acids) present
on H/K-ATPase Pump. It inactivates pump irreversibly. Acid secretion is resumed when new pumps are
synthesized and inserted into membrane.
