Teacher’s Perceptions
The following are perceived perceptions which contribute to the reading difficulties of
students based on their teachers’ response. As a respondent, you are requested to
answer the questions truthfully.
Part I
A. Physical Health Problems
1. How does the child feel right now? What makes him/her feel that way?
2. Does the child stay with parents? If not, can you tell why?
3. To whom/where does the child spend most of his/her time?
4. Does the child stay more frequently with his/her friends?
5. Is there something frightening that happened/happening to the child? How
does it affect to him/her?
Part II
A. Student’s Interest
1. How does the child feel about reading?
2. What motivates the child to read a book or story?
Part III
Alphabet and Phonemic Awareness
1. Does the child know all the letters in the alphabet?
2. Can the child produce the sounds of the letters of the English alphabet?
3. Can the child identify the vowels and the consonants in the English alphabet?
4. Can the child identify the uppercase and the lowercase of each letter of the
English alphabet?
5. Can the child blend the different sounds in the alphabet?
6. Can the child read sentences?
