I am a fourth year graduating student of Bachelor of Science in Information and Technology as

confirmed by the Office of the University Registrar. To complete the requirements of the
program, I need to enroll in IT 353 Strategic Management in IT on a Tutorial basis because I
didn't take this subject last semester due to not being approved for special class.

I am therefore, respectfully requesting your good office to allow me to enroll in the above-
mentioned subject/s.
