AFFIDAVIT OF LOSS

I, MARANATHA CARMELA ALLADO, of legal age, Filipino, single and a

resident of Davao City, upon my oath do hereby depose and say:

1. That I am a holder of the following Automated Teller Machine (ATM) Cards

and Identification Cards;

 BDO ATM Savings Account No. 001730412899;

 BDO ATM Savings Account No. 000860908909;
 DBP Payroll ATM Card with Account No. 915 5499 158;
 LTO Non- Professional Driver’s License;
 Professional Regulations Commission (PRC) ID with No. 0828211;

2. That I placed said identification cards inside my wallet for safekeeping;

3. That on or about 11:30 p.m. on August 10, 2023, I accidentally left my wallet
containing the above-described ATM and Identification Cards inside the taxi.
That I was not able to get the plate number of the taxi or the name of the
driver as I immediately left the taxi;

4. That I exerted efforts to recover said ATM and identification cards but my
efforts proved futile as the same could no longer be found despite diligent
search. Hence, I considered the same lost for all intents and purposes;

5. That I execute this affidavit to attest to the truth of the foregoing facts and for
whatever legal purposes.
