FALL HAZARD MANAGEMENT REQUIREMENTS GUIDE

A DESIGNED ACCESS WAYS / WORK SURFACES
A-1 PERMANENT PLATFORMS No fall-arresting equipment is required when working on a platform / scaffold with proper fall protection (handrails, midrails, etc.)
APPROVED SCAFFOLDS When employees perform work outside of the guarded area, lanyards must be secured to an anchor point unless specified by the project.
A-2 STAIRS Handrails must be used on stairs. Do not carry materials using both hands.
FIXED STEP PLATFORMS If task is to be performed while on these designed access ways, the lanyard must be secured to anchor point.
CAGED LADDERS < 9 METERS (30 FT).
FIXED LADDERS < 6 METERS (20 FT). Tools or other material must not be carried by hand while on ladders or continuous belt carriers.
CONTINUOUS BELT CARRIERS
Fall-arresting equipment must be provided that meets ANSI or other in-country regulatory agency requirements, such as a rail system mounted on

the ladder or a retracting device mounted above the ladder.
FIXED LADDERS NOT CAGED > 6
Employees must not climb these ladders using alternating lanyards.

PORTABLE EXTENSION / STRAIGHT
A-4 Extension / straight ladders should be secured to fixed structure or anchor point.
LADDERS
Although not required, if possible, portable step ladder must be secured to fixed structure or anchor point.
PORTABLE STEP LADDERS
Tools or other materials must not be carried in hands while on ladders.
PORTABLE STEP PLATFORMS
If tasks are performed while on portable ladders or platforms, fall protection is required.
B CLIMBING
HORIZONTAL TRAVERSE THROUGH while traversing, climbing, descending, or working in a pipe rack, the fall-arresting technique to be used is to secure a lanyard to a suitable anchor

C ROOFS (TANK AND BUILDING)
Approved hand and midrails around the roof and / or work area are considered approved fall prevention. Harnesses and Lanyards are not required as log as
employees are inside the handrails.
Employees are not required to wear harnesses and lanyards if the following conditions exist:
C-1 FLAT ROOFS ♦ The structural integrity of the roof is known and acceptable, and work does not occur within six (6) feet (1.8 meters) of the roof’s edge.
♦ The work area is barricaded or roped off.
♦ Employees are traveling directly from the point of entry to the work area.
For all other situations, including working within six (6) feet (1.8 meters) of an unguarded roof’s edge, an approved fall arrest system must be used.
SLOPED ROOFS (INCLUDING CONE
C-2 Fall protection must be utilized at all times.

D CRANE
FALL PROTECTION IS NOT REQUIRED WHILE ACCESSING DECK OR CAB PROVIDED:
♦ Designated access ways are used to access the deck and cab.
♦ Tools or other material are not carried in hands while climbing onto or descending from equipment.
FOR WORK ON A CRANE (INCLUDING DECK), USE OF A FALL ARREST SYSTEM IS REQUIRED WHEN THE FALL POTENTIAL EXCEEDS SIX (6) FEET (1.8 METERS).
E EXCAVATIONS
EXCAVATIONS REQUIRE THE FOLLOWING:
♦ The work area must be barricaded or roped off.
♦ Employees are traveling directly to the point of entry for the excavation.
When working within six (6) feet (1.8 meters) of the edge of an unguarded excavation six (6) feet (1.8 meters) deep or greater, an approved fall arrest system must be used.
