CT Brain Plain

History

Case of SDH - status post right sided decompressive craniectomy

Technique

Serial axial sections of brain were made from base of skull to the vertex without
contrast.

Observations

Status post right fronto-parieto-temporal craniectomy.

Soft tissue swelling, emphysema with a drain seen at the operative site.

There is mild effacement of the right lateral ventricle.

Rest of the ventricular system and sulci are prominent.

There are periventricular white matter hypodensities.

Residual thin sub-acute SDH of thickness 3 mm seen under right temporal


convexity.

Residual thin sub-acute SDH of thickness 4 mm seen under right frontal


convexity.

Small air locule seen in the right frontal subdural space.

Small hemorrhagic contusion seen in right temporal lobe.

Chronic infarcts seen in right putamen, right thalamus and let cerebellar
hemisphere.

The gray white differentiation is maintained.

The brainstem and basal cisterns appear normal.


Cerebello pontine angles appear normal.

The sella and parasellar regions are normal.

The basal ganglia, thalami and capsular tracts appear normal.

There is sphenoid, bilateral ethmoid and maxillary sinusitis.

Impression

Status post right fronto-parieto-temporal craniectomy.

Soft tissue swelling, emphysema with a drain at the operative site.

Mild effacement of the right lateral ventricle.

Diffuse cerebral atrophy.

Small vessel ischemic disease.

Residual thin sub-acute SDH under right temporal convexity.

Residual thin sub-acute SDH under right frontal convexity.

Small air locule in the right frontal subdural space.

Small hemorrhagic contusion in right temporal lobe.

Chronic infarcts in right putamen, right thalamus and let cerebellar


hemisphere.
