Dear Parents/Guardians:

I am writing to request your consent for your child, ____________________________

to participate in a props project for the upcoming Contemporary Arts Theater
because this Performance task will be submitted as partial requirement for
Contemporary Arts 4th Quarter.

The purpose of this letter is to inform you that your child is expected to participate
on this Props making.

The following are the details regarding the schedule and location of the activity
said:

Location: Villaluningning Clubhouse Talon Singko

Time and Date: June 13 to June 26 at 3:00pm to 5:30pm ONLY

To ensure the safety and well-being of all participants, we have taken the
necessary precautions.
1. The venue for the props making will be held in a private subdivision
2. There will be a supervision of a Parent
- Mr and Mrs. Mondejar

Regards to this by signing this consent form, you are acknowledge and agree to
the following:

1. We are not responsible of your child after the said time. Your child is no
longer part of our responsibilities even the school and the teachers and
students who are incharge of the play.

2. You understand that the props making will span a duration of two weeks
from June 13 2023 that will end on June 26 2023 with various materials and
tools as part of the project.
3. Included in the props requirement, We ask for your support by giving Php
130.00 that will be used as a budget for doing props:
• List down below are the materials needed for the Theater Play
• Costume
• Backdrops
• Props
- Weapons
