AFFIDAVIT

I, P.NIROSHA, aged about 26 years, W/o of D.Prasantha Kumar Reddy, Indian, and Hindu,
residing at Sathu village, Rallabudugur post, Santhipuram Mandal, Chittoor, Andhra Pradesh,
do here by solemnly affirm and state as follows’-
1. That I, the deponent and am seeking this affidavit for me for higher studies/Government
sector job purpose under Economically Weaker section category (EWS)
2. I state that I and my family doing agriculture and getting income of Rs. 90000/- (Rupees
ninety thousand only) per annum from all sources. I am belonging to Indian Hindu,
“REDDY” (O.C) which comes under OC and belongs to economically backward and weaker
section. I sate that I belonged to “REDDY” (OC), and need the above certificate for
educational or job propose.
3. That I, the deponents undertake to furnish the following documents to the concern
authorities and the said documents may be treated as a part of the affidavit: -
a) Family Income Certificate
b) Proof of Residence.
c) Original Birth certificate of the student.
d) Copy of the latest Aadhar card of all family members.
e) Number of family members residing in the house with their names.
f) Status of Residence: - Owned or Rented (own)
g) LPG Connection copy (self-attested)
4. That I, the deponent am having health/life/or other policy in my name or in the name of my
child/children/ spouse. I, the deponent am paying annual premium of Rs. _____________
towards the said policy/policies (Self attested copies of Policy/receipt of premium paid to be
attached) N.A
5. That if any statement made by me, the deponent in the affidavit or otherwise to the concern
authorities while seeking admission, on a seat reserved for Economically Weaker section
turns out to be false, then the admission of the deponent’s ward will be considered as
cancelled. I, the deponent will not arise any claim for seat either against reserved quota. I, the
deponent further undertake to pay the concern authorities the fees payable by the student of
general category from the date of admission till its cancellation and shall also be liable to be
prosecuted in accordance with law.
6. That I, the deponent further undertakes to intimate the concern authorities regarding any
enhancement in my income or my family income because of which I, the deponent cease to
be covered by the definition of Economically Weaker section of society as defined by the law
(RTE) or any other law or rule from time to time, and will pay the fee as applicable to general
category student from the date of their exclusion from EWS category. I will also be liable for
legal action in case of any of false information is given by me.
