1. Course Number : NCM 100n
2. Course Name : Theoretical Foundations in Nursing
3. Course Description : This course deals with nursing theories as applied to nursing practice on the aspect of the metaparadigm:
person, health, environment and nursing. Likewise, it includes other theories relevant to nursing. The learners
are expected to use these theories as basis and guide in nursing practice

16. POLICIES AND GUIDELINES

1. Only officially enrolled students are allowed to attend their classes.


2. All students are required to attend their classes regularly.
3. Attendance is counted from the first regular class meeting.
4. Punctuality is a must to all students.
5. A validated student identification card must always be worn by all students while attending classes.
6. Any form of dishonesty shall be dealt with accordingly.
7. Cheating is strictly prohibited. Any form of academic dishonesty shall be dealt with accordingly. Grave offenses such as
plagiarism and outsourcing of major requirements, final requirements, theses, dissertations, capstones, designs and similar
others, are automatically elevated to OSA/SCC/USCC for disciplinary action.
8. Students who incur absences equivalent to more than 20% of the course hours required shall be dropped from the course
provided he/she has not taken any exam.
9. Should a student nurse be absent from his/her class, he/she has to secure and fill up an excuse slip duly signed by
parent/guardian (supported by a medical certificate from the University Physician, if student is sick) and to be signed by the
subject teacher/s, College dean and to be submitted to their respective subject teachers or Clinical Instructors thereafter. A
student shall be given three (3) days grace period in the submission of the excuse slip otherwise the absence will be considered
unexcused.
10. Only students with valid examination permits are allowed to take the periodical exam as scheduled.
11. No special quiz shall be given to any student who comes in late or absent during classes.
12. Honesty is called for at all times.
13. Students who missed a major/periodical examination shall be allowed to take a “special examination” within ten (10) days after
the scheduled periodical examination as per OPM 6.11
14. Special examination shall be given only for the following reasons (with respective documentary evidence):
● Medical emergencies
● Death in the family
● Fortuitous event(s)
● No examination permits
● Participation in official school activities
● Schedule of exam in conflict with religious obligations as per CHED Memo dated November 15, 2010.
15. A student who is found guilty of false claims on any of the reasons listed in number 14 shall be given a grade of zero in the
missed exam.
16. In the event that the student fails to take the special examination as officially scheduled, there shall be no more
reconsideration.
