OSCE CHECKLIST

STATION 1
Introduce self Good day *insert proctor name*, I am
Dr. De Lara, I will be your physician for
today.
State purpose I will ask some questions regarding your
complaint. Please answer as honest as
possible so that we could give you the
proper care you need. All of these will
be confidential as part of the patient’s
confidentiality.
Identifying Let’s start with your full name, age,
data birthday, and address.

feel these symptoms?

condition or disease? Eg. Hypertension,

medical condition or diseases including

• Skin: rashes, discoloration, hair,
moles
• Head: headache, head injury,
dizziness, lightheadedness
• Eyes: vision, eyewear, redness,
tearing, spots, specks
• Ear: hearing, tinnitus, vertigo,
earaches, discharge
• Nose: colds, nosebleeds,
• Throat/ Mouth: teeth and gums,
sore tongue, drying, sore throat,
hoarseness.
• Neck: swollen, stiffness
• Breast: lumps, pain, discharge
• Respiratory: cough, wheezing,

• Cardiovascular: BP, heart rate,
orthopnea, PND, edema
• GI: swallowing, heartburn,
appetite, BM, Jaundice

menstruation, discharge
• Musculoskeletal: stiffness,
arthritis, tophi, movement

• Neurologic: headaches,

• Hematologic: anemia, pallor,
bruising, bleeding
• Endocrine: thyroid most common

STATION 2
Cardiovascular • Introduce self • Xanthomata-
